How to download FAMILY AFFIDAVIT DOCUMENT from Indian Embassy website in Kuwait?

Since I want to bring my family here from India, I need to get the Family Affidavit Form. As an additional query can anyone provide me the name(s) of concerned agencies in Kolkata(Calcutta) who use to execute the Family Visa Processing? Thanks & regards
