I have come to know that I would need my original birth certificate to have my dependent visa stamped in Kuwait, However that has been misplaced during the Iraq Invasion. I was born in Kuwait.
I had the same problem and just sorted it out last week.
If you have your old Kuwait civil Id, the job is done!
Go to Maidal Hawally (Birth and Death Registration - Ministry of Health). Meet the superindentent (Once you enter, take a left and straight down the hallway at the left end), with your old civil id copy and your passport copy.
He (barely speaks English, but nice guy and he is approachable) basically will sign it and ask to give it to the concerned department. If its 1900-1990, Its just opposite his office.
Will take couple of days to get it.
Best way is to get hold of one of the Bangaldeshi guys, pay him 3-5 KD and he will call and inform when it is ready.
If you do not have the civil Id, it might be a bit tough. But definitely worth going to the office and speak with one of the Bangladeshi guys. They are quite helpfull.
